Given the growing number of people with heart disease, the Municipality of Vila Franca de Xira and Vila Franca de Xira Hospital have joined forces to create a program to rehabilitate citizens after suffering an Acute Myocardial Infarction.
The program has several activities related to nutrition, physical exercise, nursing support, medical visits and has been achieving great recovering and improvement results for its participants.
Vita Salutis, a digital platform developed by Take The Wind, has been a part of this pilot experience as a digital solution that allows citizens to monitor their own health values and achievements and in addition to getting feedback from their healthcare providers.
